AFUHSD COVID Submissions

Welcome to the AFUHSD COVID-19 Documentation submission site. All information sent via this website is secure and won’t be used for anything except notification purposes.

This form is being used to collect one of 3 documents:

● Proof of COVID-19 vaccination

● Proof of a positive COVID-19 test within the last 90 days

● Proof of a negative COVID-19 test (must be dated 5 full days after the exposure)

Both the student ID and student email must match for the form to go through.

The person filling out the form, and the student, should receive an email confirming your submission. Both emails will also receive notification in 1-2 business days that their submission is approved.
